Documents may be presented on this page in one of both of two formats:

1. Portable Document Format (.pdf) . This format cannot be  processed electronically but must be downloaded and printed and then filled in.
2. Word 1997-2003 Format (.doc) . This format can be processed electronically before printing.

Please note that Apple Pages, Open Office, Libre Office and most word processing applications can accommodate .doc format. It is most unlikely that Microsoft Publisher formats will be available—it is a proprietary format and few desktop publishing applications can open it.

You should also note that depending on the add-ons and/or extensions implemented in your browser the file selected may open in your browser or it may open a dialog requesting a download destination.
